S02: Understanding the Clinical, Ethical, Legal, and Business Models of Accountable Care Organizations

Accountable care organizations (ACOs) have the potential to drastically change the way medicine, including psychiatry, is practiced in the general hospital. ACOsrequire that providers balance cost-effective care with more traditional values such as patient autonomy. Most psychiatrists have not received training in this new model of care. By highlighting core clinical, ethical, and business concepts, our workshop will provide participants with a practical approach to navigating the ins and outs of ACOs.

This workshop will provide a comprehensive overview of ACO business models and will clarify ethical and clinical dilemmas that arise. In light of this, the workshop will present clear practical strategies for the successful management of patients within an ACO. In addition, we will present core concepts and strategies for presenting and understanding this material that may be employed in teaching trainees and colleagues about these important changes in our healthcare system.
Understanding the business models of ACOs

John Taylor, MD, MBA, Massachusetts General Hospital and McLean Hospital
Dr. Taylor will discuss core financial concepts that are key to understanding ACOs. Furthermore he will discuss how the realignment of financial incentives and the focus on quality and outcomes measurement will affect the role of psychiatrists. He will offer a clear conceptual model for understanding and teaching this new model of care.

Understanding ethical dilemmas faced by ACOs
Amanda Green, MD, MA, Massachusetts General Hospital and McLean Hospital
Dr. Green will describe the unique ethical dilemmas that can arise in an ACO and she will discuss how we can successfully integrate both old and new values in our patient care. She will provide a useful model for understanding and teaching about complex cases.

Lessons learned from a Medicare CMS demonstration project: Clinical pearls
Rebecca W. Brendel, MD, JD, Massachusetts General Hospital
Medicare demonstration projects seek to provide coordinated care to high utilizers of medical and psychiatric care with the goal of both improving outcomes and decreasing costs. Dr. Brendel will share clinical pearls from her recent work on this project and provide strategies for successful clinical outcomes as well as important teaching points.

Clinical and systems implications of health care reform
Paul Summergrad, MD, Tufts Medical Center
Dr. Summergrad will discuss how these changes will impact psychiatric practice in the future. He will offer a clear and practical model for synthesizing, understanding and teaching about the ramifications of upcoming changes in the future of psychiatric care.

Chair and Moderator:
Ted Stern, MD, FAPM, Massachusetts General Hospital
Dr. Stern will chair and moderate the panel. He will assist in illustration of the clinical, business and ethical issues that arise in an ACO through the use of case examples and his experience in the practice of psychosomatic medicine.
