S03: Treating Both Heart and Mind: Toward Integrated Care for Cardiac Patients with Psychiatric Co-Morbidities

Lawson Wulsin, M.D.: How epidemiologic findings could guide development of integrated care for Metabolic Syndrome Patients; 3. Bruce Rollman, MD, MPH.: Blended Collaborative Care for Treating Depression and Co-morbid Heart Failure; 4. Hochang Benjamin Lee, MD.: Toward Integrated Care for Advanced Heart Failure Therapy Patients.

Psychiatric co morbidities are common among patients with cardiovascular disease and present major barriers in delivery of quality of cardiac care. Integrated care promises to address both physical and behavioral aspects of cardiac care and improve the efficiency and quality in comprehensive health services for patients with co-morbid cardiac and psychiatric conditions. In this symposium sponsored by the Cardiovascular Psychiatry SIG, each presenter will describe the current efforts to guide and develop integrated care for cardiac patients based on their latest study findings. Dr. Huffman will present the findings from the recently completed MOSAIC trial, a prospective trial of a collaborative care program to identify and treat depression, generalized anxiety disorder (GAD), and panic disorder (PD) among patients admitted to the hospital for an acute cardiac illness. Dr. Lawson will present the recent analysis of the Framingham Heart Study and how the findings could identify high risk group for metabolic syndrome and guide the implementation of a stepped care at the VA system. Dr. Rollman will present the latest findings from his trial of collaborative strategy for treating post-CABG depression, and his team's newest study testing a "blended" collaborative care strategy for treating both depression and heart failure. Dr. Lee will provide data and discussion on how integrated model affects advanced heart failure patients who are either candidates for or recipients of cardiac transplantation or left-ventricular assist device. Finally, Dr. Söllner will provide European perspective on the aforementioned efforts to develop integrated care models for cardiac patients and discuss the worldwide trend in health care reforms and new organizational arrangements focusing on more coordinated and integrated forms of care provision.
